Lymphedema is defined as swelling that does not resolve with time or elevation. Lymphedema can develop for a variety of reasons across the lifespan, or it can be present at birth. Primary lymphedema is swelling that occurs due to a person’s decreased lymphatic capacity, whether it is observed in utero or after birth, or if it onsets later in life. Secondary lymphedema is typically due to cancer related surgery, trauma, obesity, and other lymph vessel damage. Chronic venous insufficiency can be an underlying contributor to lymphedema, as well.
The cause of lymphedema is important in planning treatment, however the goal of all treatment is to normalize the size of the swollen area to increase comfort, improve skin integrity, and support local immune functions. Complete decongestive treatment includes measurements to track progress, carefully planned manual lymphatic drainage, skin care, exercise, compression wrapping, and compression wear application. Lifetime control of lymphedema includes daily application of compression garments.
